Travel Health Questionnaire

If you require any vaccinations relating to foreign travel please contact us and we will advise how best to arrange these.  This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.  There is further information about countries and vaccinations required on the links below

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  This is because not all travel vaccinations are included in the services provided by the NHS.

Private Examinations

Certain services and examinations are not covered under the NHS and will attract a fee.  These have been recommended by the British Medical Association.

Medical Certificates

The Doctor will normally need to see a patient before issuing a  medical certificate.  For the first week of an illness a patient may obtain a self certified certificate (SC1) from the Receptionist. 

This can be completed by the patient without seeing the Doctor.  If you need a private certificate, there will be a charge for this.

Parkside Surgery is committed to assist with the challenges faced by people with communication needs in a health care setting. This includes people who are deaf, blind, visually impaired, have speech or language difficulties, don't speak English as a first language, have a learning disability, or have any other communication need.

Being able to communicate effectively with patients is an essential part of providing safe, effective care, and we are committed to making that as easy as possible for all patients registered with Parkside Surgery.

This includes ensuring that proper support is put in place for people who rely services to assist with communication.

1.       Ask if you have any communication needs.
2.       Record those needs on your health records.
3.       Ensure that those needs are easily visible on your records.
4.       Share those needs with other services if you are being referred.
5.       Make sure those needs are met as part of ALL care.
