Registering with Parkside Surgery is a simple process. You will need to complete our Registration Form available at reception or on this website, you will also need to provide photographic identification and your medical card
If you have lost your medical card or have lived outside the UK, the Receptionist can give you— Form No. HS200 to request a new card or Form No. HS22X if you have lived outside the UK
As part of our service you will be invited to make a 20 minute appointment for a registration health check with the practice nurse,
